Required USPS forms
By law, virtual mailbox providers must collect identity and address verification. After your application is approved you’ll need to provide:
- Completed USPS Form 1583
- Government-issued photo ID (front and back)
- A second proof of address if required (e.g., utility bill or bank statement)
You’ll be able to securely upload these documents inside your Usufzy Mail customer portal.
